Please submit your enrolment forms along with Birth Certificate and Medicare Immunisation statement as soon as possible.
If you are a new family wishing to enrol, we require proof of address. This must be the most current Council Rates Notice (if you own the home you are living in) or the current Lease agreement (if you are renting the home you are living in) along with two other forms of ID (eg: utility bills, drivers licence etc.) with your name and address.
Please see the school office staff with the required paper work and once your address has been verified you can obtain the enrolment pack and start the enrolment process.
*Please note that Deer Park West Primary School is zoned – determined by the Department of Education. Families have automatic admission if they live within our school zone.
